Registration forms can be obtained at the Department of Motor Vehicle locations and many major pharmacies throughout the state of Utah. A registry form can be downloaded under Donor Registration/Ways to Register. All registration forms should be mailed to:

All those who register by mail will receive an email or postcard verifying that their information has been entered into the registry. Each individual may look up their own registry information after receiving a Registration ID. Those who register via the Internet will be given a Registration ID following on-line registration.

• Mail-in Registration
  Registration may be done through the website or by mailing in a registration form. For a copy of the printable registration form, click here.
  

• Online Registration
  To register online, use the "Online Donor Registration" option on the menu.
  

• DMV Registration
  If you indicate you wish to be a donor at the time you obtain your driver license, a letter will be sent to you explaining how this information is a part of the registry.
